About The Position
Alhambra Post Acute is seeking a compassionate, organized, and resident-focused Social Services Designee to join our team in Martinez, California. In this role, you'll be a vital advocate for our residents and their families, helping address social, emotional, and psychosocial needs while supporting quality of life, care planning, and successful transitions throughout the healthcare journey. Requirements
- Social Services Designee Certification
- Strong communication and interpersonal skills
- Excellent organization, time management, and problem-solving abilities
- Ability to build positive relationships with residents, families, staff, and community partners
Nice To Haves
- Skilled nursing or healthcare experience
- Experience with PointClickCare (PCC)
Responsibilities
- Provide medically related social services to help residents achieve and maintain their highest practicable physical, mental, and psychosocial well-being
- Assess residents' social, emotional, and family needs and contribute to individualized care plans
- Offer emotional support and counseling to residents and families coping with illness, disability, grief, and life transitions
- Assist residents and families with care planning and healthcare-related decisions
- Coordinate discharge planning with the interdisciplinary team, including referrals for community resources, services, and equipment
- Schedule outpatient appointments and ancillary services including dental, optometry, podiatry, and psychiatric visits
- Connect residents and families with community programs and support services
- Assist with personal shopping requests and follow up on lost or misplaced belongings
- Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements
- Collaborate with nursing, therapy, administration, and community agencies to support resident success
