About The Position

This role is for a Social Services Coordinator at Charter House. Requirements

• Bachelor's degree from an accredited college or university in Social Services or Social Work preferred.
• Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program.
• If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
• Must be able to work in collaboration with others.
• Knowledgeable about Medicare and other third-party payors.
• Good communication and human relation skills.
• Strong problem-solving skills.
• Ability to collect, compare, sort and prioritize information to be used in the decision-making process.
• Must be able to adapt to unpredictable situations.
