Social Services Coordinator
About The Position
Bishop Spencer Place is seeking a Social Services Coordinator to join their team. This role is responsible for assessing the psychological, social, emotional, cultural, or financial needs that impact a resident's health and recovery. The coordinator will provide professional social work services, including supportive counseling and clinical knowledge, to assess patient and family needs during illness, crisis intervention, and post-discharge planning. This position collaborates with the healthcare team to develop transition plans and address needs promptly. The role also involves participating in program planning and development to meet customer needs within the healthcare system and community, and assisting the Social Work team. The Social Services Coordinator will also assist the multidisciplinary team and patients with discharge arrangements, work with community agencies to ensure patients have the correct services before discharge, assist patients with transportation arrangements, and participate in treatment team meetings.
Requirements
- Less than 1 year of applicable experience
- Bachelor's Degree

Responsibilities

- Working with the multidisciplinary team to provide services that impact patient health and recovery.
- Providing assistance to the Social Work team.
- Assisting the multidisciplinary team and patients with making discharge arrangements.
- Working with community agencies to ensure patients have the correct services in place prior to discharge.
- Assisting patients in making transportation arrangements.
- Participating in treatment team meetings.
