Requirements

High school diploma or GED
3 years of experience serving older adults or people with disabilities, or in social services in a healthcare setting

Responsibilities

Build relationships with residents and community members to assess needs, provide guidance, and connect individuals with appropriate support services
Maintain ongoing contact with residents who may benefit from additional assistance, including those returning from rehabilitation or hospitalization
Support residents and families through care transitions by helping identify appropriate housing and service options
Maintain resource directories, track program outcomes, and complete required compliance, funding, and regulatory reports
Develop and coordinate educational, wellness, recreational, and enrichment opportunities that promote resident engagement
Create resident communications, including newsletters and community updates, while serving as a liaison to resident committees and groups
Collaborate with community leadership to support resident eligibility, documentation, and compliance with lease and housing program requirements
