About The Position

The Social Services Coordinator will be responsible for conducting psychosocial assessments and program intakes, gathering and tracking client data, ensuring client paperwork is updated, communicating with clients, assisting clients with accessing benefits and services, providing crisis intervention, assisting with staff management, coordinating staff schedules, handling community inquiries, representing the program at events, covering an emergency cell phone, planning and executing social activities, assisting with social service programs and events, supervising interns, preparing surveys, organizing client files, and assisting with data collection and reporting. Flexibility in working hours, including evenings and weekends, is required.

Requirements

  • Minimum Bachelor's in Social Work, Non-Profit Management or related field, and/or 1-2 years social work experience if BA is in an unrelated field.
  • Valid Florida Driver's License
  • First Aid/CPR Certification
  • Ability to perform each essential duty satisfactorily.
  • Confidentiality Agreement is required and must be signed upon commencement of service.

Responsibilities

  • Conducting psychosocial assessments and program intakes for clients.
  • Gathering and tracking client data for programmatic outcomes.
  • Ensuring client charts are thoroughly documented and paperwork is updated regularly and in compliance.
  • Communicating with program clients to ensure satisfaction, address challenges, collect data, inform of policies, provide support, and make referrals.
  • Assisting clients to access appropriate benefits, basic needs, medical, mental health, and substance use treatment providers.
  • Providing crisis intervention for immediate housing needs, medical, mental health, and substance use issues.
  • Assisting the Director of Social Services with direct care staff management, including interviewing, hiring, orientation, guidance, reviews, coaching, and disciplinary action.
  • Assisting the Social Services department with scheduling and coordinating staff for in-home visits and center-based locations.
  • Handling community inquiries on agency services and making client referrals to community resources.
  • Representing the program at occasional community events and/or presentations.
  • Coverage of emergency cell phone on a bi-weekly basis or as needed.
  • Planning and executing monthly Social & Activity Group for the program.
  • Assisting the Social Services Administrator with social service related programs and events, nutrition education classes, parent groups, sibling groups, and social work team meetings.
  • Assisting with supervision, task management, and instruction of social work interns.
  • Preparing parent surveys.
  • Organizing and maintaining client files and staff as required by the County grant.
  • Assisting with data collection, monitoring, and reporting requirements related to enrollment and social services.
