About The Position

Volunteers of America National Services (VOANS) is seeking a Social Services Assistant to serve as a liaison and role model to the community for the facility. This role represents the Social Service department, other care center staff, interdisciplinary team members, and the facility as a whole to the community. This is an on-site role. Requirements

  • High school diploma or equivalent education required
  • One year experience in a long-term healthcare setting
  • Strong communication and organizational skills

Responsibilities

  • Coordinates resident transfers and discharges, including preparing cases for review, serving as transfer/discharge planner, arranging meetings, documenting plans, and maintaining a list of agencies that can assist residents and families.
  • Completes assessments of residents for Care Planning, MDS, CAAS, PPS, and other required documents. Identifies, provides, and/or coordinates for the medically-related social, psychological, and spiritual needs of the residents.
  • Develops Care Plans and attends Interdisciplinary Care Conferences for residents, serving as a resident advocate, coordinating invitations, referring to manuals, implementing case management, counseling, or support, and maintaining confidential records.
  • Initiates reporting and documentation in the medical record, including electronic medical records, completing and updating assessments and documentation per policy and regulations, preparing written summaries, and notifying admissions coordinator of payer change status.
  • Develops and promotes the department through marketing efforts, maintaining a Community Resource and Advocacy Agency File, visiting referral agencies and hospitals, providing community resource information, and planning/presenting sessions on aging and long-term care.
  • Serves on committees as required by the Social Services Director.
  • Attends/coordinates family council/educational meetings and resident council.
  • Serves as rights advocate liaison to the facility and coordinates posting of all Federal and State required material for resident and family advocacy.
  • Advises the Director of Social Services of major problems or concerns of residents or their family members, recommends solutions, and follows the concern-problem resolution process.
  • Ensures that residents have necessary and adequate clothing, health aids, etc.
  • Coordinates resident admissions, including taking pre-admission calls, managing a waiting list, offering tours, conducting pre-admission screenings, meeting new residents, completing admission documents, and advising residents and families of advance directive policies.
  • Develops statistics on department operation and maintains admission/discharge/referral/inquiry statistics.
  • Assists with education to residents, families, staff, etc. on topics such as bill of rights, vulnerable adult, advance directives, etc.
  • Assists with and coordinates information and referral for residents (e.g., finances, counseling, support services, mental health, etc.).
  • Attends required in-services and completes assigned online modules.
  • Performs other duties or special assignments as directed by the Director of Social Services.
