Requirements
- Medicaid application experience
- Experience with social work in a LTC setting
- Compassion for an underserved population
- Ability to work effectively as a team
- Excellent communication skills
Nice To Haves
- Degree in an associated field
Responsibilities
- Provide assistance in caring for residents by ensuring care plans meet their needs.
- Communicate effectively with residents and their families.
- Work under the supervision of the Social Services Director to ensure compliance with current state, federal, and local standards.
