Requirements

  • High school diploma or equivalent required.
  • Ability to work effectively in a fast-paced environment while managing multiple priorities.
  • Strong communication and interpersonal skills with the ability to build positive relationships with residents, families, and team members.
  • Ability to assess social, family, psychological, and emotional needs and contribute to appropriate care planning.
  • Compassionate, empathetic, and resident-focused approach to care.

Nice To Haves

  • Previous experience in a skilled nursing facility and familiarity with PointClickCare (PCC) preferred, but not required.

Responsibilities

  • Collaborate with residents and their families to establish goals and develop plans that promote independence, self-care, and overall well-being.
  • Provide medically related social services to support each resident's physical, mental, and psychosocial needs.
  • Assess social, family, psychological, and emotional needs and assist with care plan development.
  • Offer emotional support and guidance to residents and families coping with illness, disability, adjustment challenges, grief, and loss.
  • Participate in discharge planning and coordinate referrals with appropriate community resources.
  • Interview residents, guardians, and sponsors to obtain necessary admission information and documentation.
  • Prepare and maintain resident identification records in accordance with facility policies and procedures.
  • Maintain accurate records of bed availability.
  • Process resident admissions, transfers, and discharges in compliance with established policies and procedures.
  • Provide admission packets, including resident rights, privacy notices, and admission agreements.
