About The Position

The Social Services Assistant provides assistance to the Social Services department to ensure all medically related emotional and social needs of patients are met in accordance with all applicable laws, regulations, and Life Care standards. We are seeking an energetic, compassionate, and highly organized Social Services Technician / Resident Services Coordinator to join our Social Services team. This is an excellent opportunity for an individual interested in healthcare, social services, human services, resident advocacy, or care coordination. A social work license is not required for this position.

Requirements

  • High school graduate or equivalent
  • Prior health care experience preferred
  • Make independent decisions when circumstances warrant such action
  • Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
  • Implement and interpret the programs, goals, objectives, policies, and procedures of the department
  • Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation
  • Maintains professional working relationships with all associates, vendors, etc.
  • Maintains confidentiality of all proprietary and/or confidential information
  • Understand and follow company policies including harassment and compliance procedures
  • Displays integrity and professionalism by adhering to Life Care’s Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training
  • Computer and documentation skills required.

Nice To Haves

  • Experience in healthcare, skilled nursing, assisted living, behavioral health, human services, customer service, or care coordination preferred.
  • Degree or coursework in Social Work, Human Services, Psychology, Sociology, Healthcare Administration, or a related field preferred but not required.
  • Social work licensure is not required.
  • Excellent communication and interpersonal skills.
  • Strong organizational and follow-through skills.
  • Comfortable communicating with residents and families.
  • Ability to prioritize multiple responsibilities in a fast-paced healthcare environment.
  • Compassionate, dependable, organized, and willing to take initiative.

Responsibilities

  • Support the Social Services Department with resident and family communication and follow-up.
  • Assist with resident concerns, grievances, personal needs, and psychosocial support.
  • Help coordinate appointments, transportation, community resources, and resident services.
  • Assist with advance directive, resident information, and departmental documentation processes as assigned.
  • Track Social Services recommendations and outstanding follow-up items.
  • Communicate with nursing, therapy, activities, admissions, and other interdisciplinary team members.
  • Assist residents and families with questions and connect them with the appropriate department or resource.
  • Maintain organized records, logs, and follow-up systems.
  • Participate in resident care and interdisciplinary meetings as assigned.
  • Provide timely, professional, and compassionate customer service to residents and families.
  • Assist with the social services program to meet the medically related social and emotional needs of patients as well as Federal, State, corporate, and division guidelines.
  • Document appropriately and timely, according to State law.
  • Assist social worker(s) in supporting patients and families through education, financial planning assistance, liaison with community agencies, etc.
  • Exhibit excellent customer service and a positive attitude towards patients.
  • Assist in the evacuation of patients.
  • Demonstrate dependable, regular attendance.
  • Concentrate and use reasoning skills and good judgment.
  • Communicate and function productively on an interdisciplinary team.
  • Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.
  • Read, write, speak, and understand the English language.
