Requirements

Experience in a SNF is ideal
Excellent communication skills
Ability to work collaboratively with residents, their loved ones, and our clinical teams
Ability to work with people of various socioeconomic backgrounds

Nice To Haves

A Human Services (or similar) degree is a plus

Responsibilities

Assist with care for our residents by coordinating with their care team ensuring their physical and emotional needs are met.
Work alongside community partners to ensure a safe transition home, when possible.
Work under the supervision of our Social Services Director to ensure compliance with current state, federal, and local standards.
