About The Position

Canyon Springs Post Acute is seeking a Full-Time Social Services Assistant to join their dedicated team. This role involves working closely with the Social Services Director to support residents and their families, ensuring their emotional, psychosocial, and discharge planning needs are met throughout their stay. The Social Services Assistant assists in planning, developing, organizing, implementing, evaluating, and coordinating the facility's social services program in accordance with federal, state, and local regulations, as well as facility policies and procedures. This position helps ensure the medically related emotional, social, and psychosocial needs of each resident are identified and addressed to promote the highest possible quality of life.

Responsibilities

  • Assist the Social Services Director in planning, implementing, and evaluating the facility's social services program.
  • Support residents and families by addressing emotional, psychosocial, and adjustment needs.
  • Participate in care planning, interdisciplinary meetings, and discharge planning.
  • Coordinate appointments and ancillary services, including optometry, podiatry, dental, psychiatric, and other community resources.
  • Assist residents and families with Medicare, Medicaid, and other financial assistance information.
  • Complete and maintain accurate documentation, reports, and social service progress notes.
  • Collaborate with nursing, therapy, physicians, and other departments to coordinate resident care.
  • Assist with referrals to community agencies and support services.
  • Help coordinate safe and effective discharge plans, including equipment and community service referrals.
  • Maintain positive working relationships with community agencies and healthcare partners.
  • Participate in orientation, in-service education, and departmental meetings.
  • Assist with department policy development, program improvement, and budget activities as assigned.
  • Maintain resident confidentiality and comply with all HIPAA, federal, state, and facility regulations.
  • Report suspected fraud, abuse, or unauthorized disclosure of protected health information in accordance with facility policies.
