Requirements

  • Must possess a Social Service Designee Certification and/or Activities Certification with the high school diploma or equivalent.
  • Have a thorough understanding of the principles of best Social Services practices.

Nice To Haves

  • A Social Services Designee Certification (SSD) from the state is preferred.
  • Previous experience in long-tern care is preferred, but not required.

Responsibilities

  • Assist the Social Services Director in the operation of the Social Services Department.
  • Interview resident/families to obtain social history.
  • Arrange transportation to other facilities when necessary.
  • Monitor resident mood/behaviors.
  • Develop and implement behavior management programs.
  • Maintain and acquire eyewear, hearing aids and dentures as well as obtaining personal items and specialty needs for the residents.
  • Oversee psychotherapeutic medications.
  • Manage room change-plans, documentation and follow-up.
  • Maintain transfer documentation.
  • Collect and execute CQI and MDS data.
  • Complete periodic progress notes.
  • Schedule and document care plan meetings.
  • Review nurses notes to determine if the care plan is being followed.
  • Maintain the confidentiality of all residents and their care to ensure resident rights, privacy and property are protected at all times.
  • Report and document resident and/or visitor incidents, accidents, complaints or allegations according to the facility’s policy and procedure using the designated occurrence reporting system.
  • Comply with safety requirements, infection control measures, and maintain a clean work environment in accordance with facility policy.
  • Ensure that work/assignment areas are clean and records, files, etc., are properly stored before leaving such areas on breaks, end of work day, etc.
  • Abide by all emergency protocols for crash carts, resident codes, fire safety, missing residents, tornado preparation, violent residents, Wander guard alarms, door alarms, and other facility policies as assigned.
  • Report any forms of abuse witnessed and aware of and taking appropriate action according to policy.
  • Follow infection control and universal precautions procedures.
  • Follow periodic cleaning schedules for equipment and returning equipment to proper area after use.
  • Ensure the facility is maintained in a clean and sanitary manner at all times.
  • Perform other duties as assigned by the Social Services Director.
