About The Position
Valley Phoenix's Social Service Workers play a crucial role in helping clients overcome challenges and achieve personal growth. Through therapeutic support, skill-building, and personalized care, you will foster meaningful relationships that empower residents to enhance their independence, overcome obstacles, and work towards their objectives.
Requirements
- Bachelor’s degree in Social Work or related field
- None
- SSW or SUDC licensure in state of practice
- CPR certification (Post-hire)
- Valley de-escalation certification (Post-hire)
Responsibilities
- Provides therapeutic behavior management as prescribed by the program-specific care plan in a residential setting
- Follows care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly
- Assists Therapists or Registered Nurses as needed to develop and review safety/crisis plans and care plans
- Develops therapeutic relationships with clients
- Reviews and assesses care plans
- Coordinates services among internal and external agencies and providers
- Meets required client care hour expectations and other organizational expectations
