Social Service Worker Residential | Adult Residential Services
About The Position
Valley Phoenix is seeking Social Service Workers for their Adult Residential Services. In this role, you will help clients turn everyday challenges into opportunities for growth through therapeutic support, skill-building, and individualized care. You will build meaningful relationships that help residents strengthen independence, navigate barriers, and make steady progress toward their goals. The Social Service Worker provides therapeutic behavior management, case management, and other services as defined by licensure, ensuring excellence in client care, fiscal management, and regulatory compliance. Essential functions include providing therapeutic behavior management as prescribed by the program-specific care plan in a residential setting, following the care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly. You will also assist Therapists or Registered Nurses as needed to develop and review safety/crisis plans and care plans, develop therapeutic relationships with clients, review and assess care plans, coordinate services among internal and external agencies and providers, and meet required client care hour expectations and other organizational expectations.
Requirements
- Bachelor’s degree in Social Work or related field
- None (experience)
- SSW or SUDC licensure in state of practice
- CPR certification (Post-hire)
- Valley de-escalation certification (Post-hire)
Responsibilities
- Provides therapeutic behavior management as prescribed by the program-specific care plan in a residential setting
- Follows care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly
- Assists Therapists or Registered Nurses as needed to develop and review safety/crisis plans and care plans
- Develops therapeutic relationships with clients
- Reviews and assesses care plans
- Coordinates services among internal and external agencies and providers
- Meets required client care hour expectations and other organizational expectations
