About The Position

Sunnyvale Gardens Post Acute is seeking a compassionate, organized, and dedicated Social Service Assistant to join their team. In this role, the assistant will work closely with the Social Services Director to support residents' emotional, social, and psychological well-being. This position is ideal for individuals passionate about patient advocacy and helping others navigate post-acute care.

Requirements

High school diploma or equivalent required.
Minimum of 1 year of experience in a skilled nursing facility, post-acute care, or assisted living environment is highly preferred.
Excellent interpersonal skills with a strong sense of empathy and patience for the elderly and medically fragile populations.
Strong administrative skills, computer literacy, and the ability to multitask in a fast-paced healthcare environment.

Nice To Haves

Associate’s or Bachelor’s degree in Social Work, Psychology, or Human Services preferred.

Responsibilities

Assist residents and their families with admission, orientation, and adjustment to the facility.
Help coordinate safe discharge plans, including arranging home health care, medical equipment, or community resources.
Maintain accurate, up-to-date social service progress notes and assessments in compliance with federal and state regulations.
Identify and resolve resident psychosocial needs, acting as a liaison between residents, families, and clinical staff.
Conduct regular one-on-one visits with residents to ensure their comfort, preferences, and rights are being respected.
