About The Position

This position is located in the Division of Community Services and Behavioral Health. The Social Service Analyst will review and assess case plans and case records of direct care staff by conducting case plan review meetings involving all parties involved in the case. The role serves as an independent neutral party and provides written comments as to the feasibility of identified goals, objectives, and time frames. Supplemental duties include providing intensive, focused support for youth who are open to FSU or CBH and are admitted to an inpatient psychiatric hospital to promote safe and timely discharges to family and community-settings. This involves coordinating with the youth, family, primary social casework staff, hospital staff, providers, and other involved individuals to develop and achieve a timely transition plan. The position is responsible for comprehensive care planning targeted at hospital discharge, facilitating family team meetings, addressing barriers to discharge, coordinating planning with the DCYF service plan and permanency goals, connecting youth and family to community-based resources, and empowering youth and family voice and choice. The role ensures warm handoffs between hospital teams and post-discharge providers and helps reduce readmissions and crises. Close communication with the DCYF primary worker and supervisor will be necessary. The position will be involved in collecting and tracking data on the care planning process, outcomes, service and support needs, and barriers.
