This position plays a crucial role in supporting young adults who have aged out of the foster care system, helping them transition into successful, independent adulthood. The Social Caseworker will manage a caseload of Foster Youth in Transition cases, collaborating with young adults and their support teams to achieve identified independent living goals. Responsibilities include conducting monthly home visits, coordinating services, preparing court reports, attending court proceedings, and engaging in activities designed to help young people reach their objectives. The role is part of the Social Caseworker - Children, Youth, & Family Services (CYFS) career progression series. Employment is subject to the terms, conditions, and policies detailed in the Personnel Policies Manual (PPM). This position requires regular in-person presence as an essential job function. The work aims to preserve and/or rehabilitate families while protecting children's physical, emotional, and mental well-being through child protection, adolescent services, placements, and/or permanency plans. Depending on the assignment, the caseworker will coordinate and make primary decisions regarding child protection practices or coordinate services and secondary-service decisions related to safety, permanency, well-being, and family preservation.
