The SLS Coordinator supports the day-to-day coordination of Supported Living Services for adults with intellectual and developmental disabilities. This position helps coordinate staffing, schedules, service documentation, authorizations, ISP meetings, participant support needs, and communication among participants, families, support staff, Regional Center representatives, and HCAR leadership. The SLS Coordinator works in both administrative and community-based settings. The position is expected to be approximately 70% office-based and 30% out-of-office, including participant homes, community settings, meetings, appointments, and service locations. The position may also provide coverage for vacations, staffing gaps, or participant service needs as assigned. This role supports person-centered services, health and safety, service continuity, accurate documentation, billing support, and participant independence while following HCAR policies, funding requirements, confidentiality standards, and applicable regulatory expectations.
