This position provides services to assigned children, youth, and families in various community settings. The role involves Intensive Home Based Services (IHBS), individual rehabilitation, and collateral support to caregivers. The Skills Trainer will develop and facilitate skill-building groups, complete assessments, and model positive behaviors. They will also coordinate with agency and community partners, develop safety plans, and provide case management functions. Documentation of Medi-Cal eligible services, participation in team meetings, and on-call rotation are also key aspects of the role. The position requires the ability to provide or arrange transportation for clients and participate in utilization and accreditation processes. Collaboration with internal programs and staff is essential for providing support and ensuring documentation meets standards.
