About The Position

Provides Mental Health Skill Building Services to individuals with serious mental illness to support community stability, independence, and successful functioning in the least restrictive environment. Provides individualized training, coaching, support, and transportation to help individuals develop and maintain skills related to health and safety, personal care, activities of daily living, medication management, community integration, and use of community resources. Services are provided in accordance with each individual's service plan, applicable regulatory requirements, and agency policies.

Requirements

Bachelor's Degree in Human Services field
QMHP or eligible to obtain QMHP-T certification
Valid Driver's License

Nice To Haves

Minimum 1 year experience in providing behavioral health services to individuals with mental health, intellectual disabilities, and/or substance use issues

Responsibilities

Provide individualized training, coaching, support, and transportation to help individuals develop and maintain skills related to health and safety, personal care, activities of daily living, medication management, community integration, and use of community resources.
Provide services in accordance with each individual's service plan, applicable regulatory requirements, and agency policies.
Assess individual strengths, needs, progress, and barriers to independent living.
Teach and reinforce daily living, health and safety, medication-management, social, and community-integration skills.
Coordinate services with individuals served, families, agency staff, providers, and community partners.
Establish effective professional relationships while maintaining appropriate boundaries.
Work independently in office, home, and community settings and maintain a flexible schedule based on service needs.
Protect confidential information and complete required records accurately within established timeframes.
Plan and coordinate activities that support each individual's service plan and identified goals.
