ESSENTIAL DUTIES AND RESPONSIBILITIES Complete a pre-assessment report for each worksite prior to the start of the program, deeming it acceptable to have participants placed there throughout the program period. Support the worksite supervisor in ensuring smooth program functioning throughout the program. Assist staff during participant orientation and registration. Consistently and closely monitor all designated worksites. Complete monitoring assessments reports on a bi-weekly basis and submit to job developer for review. Observe and monitor worksites on a weekly basis ensuring that they are following DYCD policies and updating weekly correspondence tracking sheet. Review site assessment rosters for each worksite assigned to them. Review participant time sheets for accuracy; maintain participant timesheets and attendance in accordance with DYCD policy and procedures. Review and ensure that each worksite has the following documents for each enrolled participant at their site: participant worksite referral, monitoring log, worksite participant assignments roster, evaluations, and weekly timesheets; ensure that worksite supervisors have copies of all these documents. Notify any changes at worksites related to participants, such as: never reported, stopped reporting, transferred, and any participant/supervisor conflicts to the Program Director and Counselor. Report all concerns and violations to supervisors and conduct follow up as needed. Obtain weekly timesheets from assigned worksites on designated dates and times set by the worksite. Towards the end of the program, Site Monitor will be responsible for the following: completion of worksite evaluations; collection of participant surveys; ensuring participant evaluations are completed for all participants, including participants who leave the program early as well as any youth who have been terminated. Follow-up on participants issues encountered throughout the program period. Assist job developer with worksite records and ensure that they are up-to-date with necessary documents, i.e. assessment reports, licenses, trip schedules, etc. Assist with any program activity deemed necessary by Program Director.
