About The Position

The certified nursing assistant position provides quality nursing care to residents; reports pertinent information to the immediate supervisor; responds to inquiries or requests for information; and assists the immediate supervisor with tasks to support department operations.

Requirements

CNA Certification from the State of Wisconsin
Knowledge of English language and grammar.
Proof of job focused competencies

Responsibilities

Giving at least the minimum number of baths/showers to residents as scheduled on assigned unit.
Ensuring that all missed baths have been given in the event of an absence, other than approved vacation time.
Documenting bath/shower information in the electronic health record, such as Point Click Care.
Communicating any resident skin issues with QA Nurse and/or DON.
Bathing all body parts, cleaning/triming fingernails and toenails.
Shampooing hair and trimming facial hair on female residents.
Shampooing hair and trimming facial hair on male residents.
Assisting residents with eating when not giving baths.
Assisting on the floor when baths are completed.
Documenting and/or reporting information regarding residents as instructed.
Assuring personal and staff compliance to all State and Federal requirements including but not limited to blood borne pathogens, hazardous materials, infection control and fire safety.
Assuring personal and staff compliance to resident's rights.
Knowledgeable of the individualized care plan for residents, and provides support to the residents according to their care plan.
Contributing to the care planning process by providing the charge nurse or other care planning staff with specific information and observations of the resident's needs and preferences.
Attending to the individual needs of residents, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, catheter care, colostomy care, prosthetic appliances, peri-care, transferring, positioning, re-positioning, ambulation, range of motion, communicating or other needs in keeping with the individuals care requirements.
Assisting in maintaining a safe, neat and clean environment. Reports environmental deficiencies to maintenance, such as call light or equipment problems.
Observing residents for changes in condition or behavior and promptly reporting these changes to appropriate charge nurse.
Providing care that maintains each resident's skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent resident, turning, repositioning and bathing in a timely manner.
Answering residents call lights promptly and courteously.
Performing various tasks assigned by the charge nurse, including intake/output recording, vitals, weighing residents, and collecting specimens.
Lifting, moving and transporting residents, using proper body mechanics or lifting devices for accident prevention.
Assisting and escorting residents for appointments such as at the beauty shop, activities, and doctor appointments.
Practicing careful and efficient use of supplies and linen and following established charge procedures for charge items.
Completing documentation on cares provided throughout shift.
Performing all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials.
Completing end of shift rounds and providing shift report to incoming shift.
Providing individualized attention, which encourages each resident's ability to maintain or attain the highest practical physical, mental and psychosocial well-being.
Maintaining the comfort and safety, privacy, and dignity of residents in the delivery of services to them. Interacting with residents in a manner that displays warmth and promotes a caring environment.
Fully understanding all aspects of resident's rights, including the right to be free of restraints and free of abuse (from staff, other residents, visitors and family). Responsible for promptly reporting to the charge nurse or administrative staff of incidents or evidence of resident abuse or violation of resident rights.
Protecting the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reporting missing or broken items to charge nurse or social services.
Assisting with orienting resident and their families to the nursing home on admission and to the unit where transfers occur.
Communicating and interacting effectively and tactfully with residents, visitors, families, peers and supervisors.
Promoting a homelike environment for the residents.
Promoting teamwork by showing dignity and respect to peers, including peers of other departments.
Promoting teamwork by working scheduled shifts at scheduled times.
Willingness to train new employees in a pleasant and respectable manner.
Willingness to complete work schedule when assigned and where assigned.
Willingness to be a part of problem-solving and QAPI groups.
Attending and participating in all mandatory in-services, unless prior arrangements are made and approval is received.
Maintaining confidentiality of all information.
Completing work within authorized time to assure compliance with departmental, state, and federal regulations and standards.
