About The Position

MYEP is seeking a Shift Facilitator to provide leadership and direct support within our Residential Program. This role is responsible for supporting adults with disabilities while providing on-shift guidance to staff and ensuring high-quality, person-centered service delivery. The Shift Facilitator leads by example, supports daily living needs, assists with community inclusion, and helps ensure documentation and care standards are met. This position requires flexibility, including some evenings and weekends, and regular use of agency software systems. In this role, you will provide direct support to adults with disabilities while also offering on-shift leadership, guidance, and coaching to coworkers. Responsibilities include assisting with daily living and personal care supports, administering medications as trained, monitoring and reporting changes in health or behavior, and ensuring documentation is completed accurately and timely. You will assist with community outings, appointments, and transportation, support daily routines and skill development, communicate regularly with supervisors regarding service delivery, and assist with training and onboarding of staff. Maintaining confidentiality and following HIPAA guidelines is required. This position involves providing hands-on support including bathing, showering, grooming, oral hygiene, dressing, toileting supports, and mobility assistance. Additional responsibilities include assisting with transfers, meal preparation, supporting individuals during meals, monitoring food and fluid intake, and supporting multiple individuals at one time. The Shift Facilitator is expected to provide direction and feedback to coworkers, help ensure quality supports are delivered, assist with staff coaching, make on-the-spot decisions to ensure safety, and promote a positive team environment.

Requirements

  • At least 18 years of age
  • High school diploma or GED
  • Valid driver’s license
  • Willing to work some evenings and weekends
  • Able to provide hands-on supports
  • Able to work independently
  • Able to problem solve in an unsupervised setting
  • Complete required training
  • Comfortably use agency software systems
