About The Position
NorthStar Services is seeking a Shared Living Provider in Northeastern Nebraska. NorthStar provides services to people with intellectual and developmental disabilities throughout Northeast Nebraska and surrounding areas. A Shared Living Provider shares their home with a participant and provides support and services to that person. This is a private contractor position. The provider is responsible for delivering and documenting habilitation, medication administration, medical care, and appointments. They also support the participant with community involvement, healthy or therapeutic meals, personal care, transportation, finances, communication, leisure activities, interpersonal relationships, family roles, reciprocity activities, and establishing or maintaining a daily routine. The provider helps maintain a supportive, safe, and healthy living environment. NorthStar supports the provider and participant through background checks and associated costs, required training, HCBS reviews, assistance obtaining an NPI number and troubleshooting in Maximus, provision-sheet verification and online payment entry, quality assurance and oversight, emergency assistance, nurse access and oversight, program and healthcare or mental health staff objectives, monthly documentation reviews and revisions, monthly monitoring of the Shared Living setting, and backup and support staff.
Responsibilities
