About The Position

This position is for a Mosaic at Home Coordinator who will empower people with intellectual and developmental disabilities to live meaningful lives in caring communities. The Coordinator acts as a bridge builder, ensuring individuals have a voice and personalized support. The role involves building relationships with individuals, families, host home providers, and direct care workers, championing individual goals and dreams, monitoring relationships between individuals and home providers, architecting personalized support plans, overseeing service delivery and contract compliance through home visits, and providing on-call support to providers and direct care workers.

• Ability to travel for home visits (can be up to 6 hours from Hastings).

Responsibilities

• Build relationships with individuals supported, their families, host home providers, and direct care workers.
• Help individuals express their goals, dreams, and needs.
• Monitor relationships between individuals and their home providers to ensure a positive and supportive match.
• Collaborate with the service planning team to create Individual Support Plans.
• Oversee service delivery and contract compliance through regular home and service environment visits.
• Provide on-call support to providers and direct care workers.
