About The Position

The Shallow Subsidy Case Manager is responsible for supporting veterans and veteran households enrolled in the SSVF program in maintaining stable housing by providing ongoing case management, monitoring rental assistance, and connecting clients to resources that promote long-term housing stability and self-sufficiency. This role involves conducting intake, eligibility screening, and reassessments, developing and implementing individualized Housing Stability Plans, and maintaining regular contact with clients. The Case Manager will coordinate and monitor shallow subsidy rental assistance, track rent portions, recertifications, and changes in client income or household composition. They will identify early signs of housing instability and intervene to prevent eviction, provide landlord mediation, and connect clients to various resources such as employment, benefits, healthcare, and legal services. Additionally, the role requires assisting clients with applications for mainstream benefits, maintaining accurate documentation in the AWARDS data system and client files, ensuring compliance with SSVF and VA standards, and participating in quality assurance activities. Collaboration with team members, housing counselors, and external partners is essential, as is educating clients on budgeting, lease compliance, and tenant responsibilities, and supporting long-term planning to reduce subsidy reliance.

Requirements

  • Strong knowledge of case management principles and housing stabilization practices
  • Ability to work with diverse populations, including veterans and individuals experiencing housing instability
  • Strong organizational, documentation, and time management skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficient in Microsoft Word, Excel, and PowerPoint
  • Must have access to a car for visits to veterans in their homes or other community locations
  • A Valid Driver's License and Insured Vehicle
  • PPD (TB test required with proof of results; no more than 1 year old)

Nice To Haves

  • Familiarity with SSVF program guidelines, VA requirements, and housing programs
  • Experience working with veterans, homeless populations, or low-income households
  • Knowledge of HMIS or similar database systems

Responsibilities

  • Conduct Shallow Subsidy intake, eligibility screening, and reassessments for program participants
  • Develop, implement, and update individualized Housing Stability Plans with goals and measurable objectives
  • Maintain regular contact with clients to monitor housing stability and progress toward goals
  • Coordinate and monitor shallow subsidy rental assistance, ensuring timely and accurate payments
  • Track rent portions, recertifications, and changes in client income or household composition
  • Identify early signs of housing instability and intervene to prevent eviction
  • Provide landlord mediation and maintain positive landlord relationships
  • Connect clients to resources, including employment, benefits, healthcare, and legal services
  • Assist clients with applications for mainstream benefits (e.g., SSI/SSDI, SNAP, VA benefits)
  • Maintain accurate, complete, and timely documentation in AWARDS data system and client files
  • Ensure all files meet SSVF and VA compliance standards, including required documentation and consents
  • Participate in internal file reviews, audits, and quality assurance activities
  • Collaborate with team members, housing counselors, and external partners
  • Attend team meetings, case conferences, and supervision sessions
  • Educating clients on budgeting, lease compliance, and tenant responsibilities
  • Support clients in long-term planning to reduce reliance on subsidy over time
