About The Position

This is an In Office/On Site position, Monday through Friday and requires on call rotation. This role operates administratively in a variety of different Service Departments. Rather than institutionalizing individuals who have these physical and mental health needs, we provide these individuals with the opportunity to live and receive services in a way honoring to them and their abilities. Service Programmers provide a fresh look at the services we provide. Within the team, you’ll join the mission of empowering people to reach individual achievement across the spectrum of life. As a core team member, you’ll be responsible for monitoring services, creating new strategies to reach goals, and coordinate services in a way that empowers people to meet and exceed their goals.   • Age Requirement: This position requires you to be at least 18 years of age.
  • Driver’s License: You’ll be responsible for transportation at times, so we need to trust your ability to be safe behind the wheel.
  • Reliable Transportation: You’ll need reliable transportation. You will also need to provide proof that your automobile insurance covers $100,000/$300,000/$50,000 liability coverage.
  • Minimum Education Requirement of High School Diploma/GED equivalent.
  • Candidates must be able to work in the U.S. without sponsorship

Responsibilities

  • Build and maintain a positive team culture through active staff engagement, regular team meetings, coaching, and ongoing support.
  • Participate in on-call responsibilities and provide short-notice coverage when needed to maintain continuity of services.
  • Oversee key resources and operational needs, including vehicle scheduling, individual budgets, representative payee coordination, medication check-ins, appointment scheduling, transportation, and other supports necessary for effective service delivery.
  • Coordinate with individuals, families/guardians, case managers, Medicaid Community Based Care Managers (CBCM), and the Interdisciplinary Team (IDT) to develop individualized plans, establish goals and outcomes, maintain authorizations and required assessments, and monitor individual progress.
  • Ensure services comply with applicable agency policies, laws, regulations, and industry standards.
  • Maintain complete, accurate, and timely documentation in the approved EMR, including service records, incident reports, investigations, individual interactions, goal progress, and changes to services or rights.
  • Advocate for individuals to have meaningful choice, independence, dignity, and control over their lives while ensuring services reflect their needs, preferences, and goals.
  • Collaborate with internal teams and community partners to strengthen resources, relationships, and opportunities available to individuals served.
