About The Position

LSS is seeking a compassionate and organized Service Facilitator to partner with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate services that promote independence, well-being, and growth. This role is within the Comprehensive Community Services (CCS) Program in Trempealeau County, WI. The position is full-time, 40 hours per week, and offers meaningful work that impacts lives every day. The compensation varies based on education and/or credentials: Bachelors: $24.20/hr, Master's Degree: $27.20/hr, Master's w/therapy licensure: $28.20/hr.

Requirements

  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or a related field.
  • Experience working with individuals, families, or vulnerable populations preferred.
  • Strong communication, organization, and relationship-building skills.
  • Ability to manage documentation, coordinate multiple services, and work independently.
  • Valid driver's license, reliable transportation, and ability to travel within the community.

Responsibilities

  • Conduct functional and needs assessments.
  • Develop and implement person-centered service plans.
  • Coordinate services and connect clients with community resources.
  • Facilitate team meetings and collaborate with families, providers, and support networks.
  • Maintain timely, accurate documentation and client records.
  • Advocate for clients and support positive outcomes through trauma-informed, strengths-based practices.
