About The Position

Services without coordination can be confusing and counterproductive - that’s why we have a Service Coordinator. The Service Coordinator efficiently coordinates services for clients with developmental disabilities to build and enhance work-related skills by utilizing the outdoors and various art mediums. This position is also responsible for the administrative aspects of services, including maintaining client files and reporting on monthly outcomes and statistics.

Requirements

  • Post-secondary education in a related field and 2 to 3 years’ experience with case management/service coordination in a similar industry or environment.
  • Exceptional oral and written communication skills.
  • Excellent organizational and analytical skills.
  • Demonstrated proficiency with Microsoft Office.
  • All employees must pass a vulnerable sector check.
  • A reliable vehicle and valid driver’s license is required for business travel.

Nice To Haves

  • Prior experience with intake processes, employability assessments, accommodations, and service plans would be considered strong assets.
  • Those with a combination of post-secondary education along with professional experience may be considered.

Responsibilities

  • Provide orientation to Prospect and program services, assess client goals and needs, schedule – coordinate and carry-out intake and planning meetings with clients and members of their support network, arrange for accommodations or assistive technologies where appropriate, determine acceptance of clients into service, develop employability assessments, service plans, employment profiles and client schedules.
  • Act as the central point of contact for the client support network, from intake to completion of service.
  • Monitor goal attainment, including soliciting feedback from other departments such as Employment Specialists, Employment Retention Specialists and Program Coordinators.
  • Support other Prospect direct service positions in working directly with Prospect clients; providing support in order to help expand employability skills development.
  • Maintain knowledge of supports and resources within the community, including financial supports, funders, mental health service providers, supports for persons with developmental disabilities, crisis supports, behavioral supports, counseling supports and education and training supports.
