About The Position

The Service Coordinator is responsible for planning, coordinating, and delivering participant-centered social, educational, health promotion, recreational, and supportive services to older adults participating in SHOPP's Community Care Network Program (CCNP). The Service Coordinator fosters participant engagement by developing meaningful programming that promotes healthy aging, social connectedness, independence, and overall well-being. Through outreach, participant assessment, program coordination, supportive counseling, community partnerships, and collaboration with interdisciplinary staff, the Service Coordinator ensures participants have access to quality services and resources that enhance their quality of life.

Requirements

  • Bachelor's degree in Social Work, Human Services, Gerontology, Recreation, Psychology, Sociology, or a related field.
  • Experience working with older adults or other vulnerable populations in a community-based, recreational, healthcare, or social service setting.
  • Experience coordinating group activities, educational programming, or community-based services.
  • Knowledge of community resources, public benefits, and supportive services available to older adults.
  • Strong organizational, planning, and program coordination skills.
  • Excellent verbal, written, interpersonal, facilitation, and customer service skills.
  • Proficiency in Microsoft Office applications and electronic documentation systems.
  • Ability to work independently while effectively managing multiple program sites and priorities.
  • Ability to maintain confidentiality and exercise sound professional judgment.

Nice To Haves

  • Experience working within NYC Aging-funded programs or similar older adult service programs.
  • Bilingual (English/Spanish).
  • Experience facilitating group activities, workshops, or wellness programming for older adults.
  • Experience recruiting and supervising volunteers.
  • Knowledge of benefits and entitlement programs including SNAP, Medicaid, Medicare Savings Programs, SCRIE/DRIE, HEAP, and housing resources.
  • First Aid and CPR certification.

Responsibilities

  • Coordinate, implement, and monitor engaging social, educational, recreational, cultural, wellness, and health promotion activities for older adult participants.
  • Conduct outreach and recruit eligible older adults into SHOPP programs and services through community engagement, presentations, and relationship building.
  • Visit assigned SHOPP sites, satellite locations, and Pop-Up Cafés on a scheduled basis to monitor programming, engage participants, and assess ongoing needs.
  • Register participants and maintain accurate enrollment records in accordance with program requirements.
  • Screen participants for benefits, entitlements, supportive services, and other available community resources.
  • Collaborate with Case Workers and other SHOPP Team to connect participants with appropriate benefits, case assistance, and supportive services.
  • Provide supportive counseling, wellness check-ins, and Telephone Reassurance calls to assigned participants as appropriate.
  • Provide information, referrals, and resource navigation to participants and caregivers, ensuring appropriate follow-up when needed.
  • Partner with Advisory Leaders, volunteers, participants, community organizations, and host site staff to develop participant-driven programming that reflects the interests, needs, and cultural diversity of the communities served.
  • Coordinate virtual programming and assist participants in accessing online activities and technology-based services when applicable.
  • Recruit, train, coordinate, and support volunteers participating in program activities and maintain volunteer records as required.
  • Maintain accurate participant records, attendance, program documentation, statistical reports, and electronic documentation within the agency's reporting systems.
  • Monitor program participation and assist in achieving contractual performance targets and program outcomes.
  • Develop and maintain collaborative partnerships with community organizations, and other agencies that enhance programming and participant access to services.
  • Assist with special events, health fairs, community outreach initiatives, and organizational projects as assigned.
  • Maintain confidentiality and ensure compliance with HIPAA, agency policies, and applicable regulations.
  • Participate in supervision, team meetings, and organizational initiatives.
  • Maintain compliance with SHOPP personnel policies, procedures, and funder requirements.
  • Complete accurate and timely documentation, reporting, and administrative tasks.
  • Support program and organizational goals and meet established deadlines.
  • Demonstrate professionalism, accountability, and responsiveness in all aspects of work.
  • Work independently and collaboratively to support team and program success.
  • Demonstrate flexibility in supporting evolving program and organizational needs.
