About The Position

The Crisis Residential Program provides short-term, intensive, round-the-clock, personalized support to help stabilize individuals experiencing a mental health crisis, aiming to prevent hospitalization. This is a direct care opportunity to support clients' mental health, independence, and overall well-being. The Service Coordinator will assess and monitor client needs, develop service plans, connect clients to community services and resources, and advocate for them as necessary. The team collaborates with existing providers involved in an individual’s treatment and works to link clients to other services offered by Sheppard Pratt or other community providers.

Requirements

  • A high school diploma or GED and 2 years of related experience OR a bachelor’s degree and 1 year of related experience
  • Willingness to obtain additional certifications and trainings as required
  • A driver’s license with 3-points or less
  • Access to an insured vehicle
  • Availability to work days, nights, weekends, and holidays depending upon need of the program

Responsibilities

  • Assessing and monitoring client needs
  • Developing service plans
  • Connecting clients to community services and resources
  • Advocating for clients as necessary
  • Assisting clients through program enrollment and orientation
  • Coordinating client medical appointments, transportation, housing, money management, etc.
  • Developing and assisting with client medication management plan
  • Obtaining and maintaining entitlements for clients
  • Maintaining accurate and complete documentation
  • Developing and coordinating transitions plans and program discharge
