Requirements
- High school diploma or equivalent required.
- Minimum 3 years in service coordination/delivery.
- Minimum 2 years in a supervisory role.
- CPR/First Aid, Article 9, and Principles of Caregiver certification (or Instructor status).
- Strong organizational skills, computer proficiency, accurate record-keeping, and problem-solving ability.
Nice To Haves
- Spanish or Russian bilingual skills are highly desirable.
Responsibilities
- Receive new referrals, manage intakes, and create personalized service plans for home and community-based services.
- Conduct regular home visits to ensure direct care quality and support standards are met.
- Partner closely with clients, case managers, direct care workers, and community partners.
- Oversee scheduling adjustments, handle incident investigations, and provide rotating after-hours on-call support.
