Pathways to Housing DC is an innovative agency based in the heart of Washington, D.C with services in Montgomery County Maryland. We believe that housing is a basic human right, and that without housing people are unable to address their mental health disorders, addictions, and other life stressors. We provide each person entering our program their own apartment, without requiring treatment compliance or sobriety as a prerequisite. We then provide each person with intensive support services to help them stay housed and begin their journey to recovery. The Assertive Community Treatment (ACT) team provides flexible, community-based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals who have a mental illness and co-occurring substance use disorders. The Service Coordinator works in collaboration with the trans-disciplinary team (including Psychiatrist, nurses, addictions counselors, and peers) and provides ACT services for all clients on the team in a compassionate and responsible manner. All services consumers are consumer-driven and recovery-focused. Successful Service Coordinators are flexible and patient. They work well in a fast-paced environment, and are independent thinkers.
