About The Position

Matthew's Place is seeking a compassionate and organized Service Coordinator to play a vital role in helping residents access the resources, services, and support they need to age successfully in place and enjoy a high quality of life. This role involves building relationships, advocating for seniors, connecting people to life-changing resources, and creating a thriving community where residents feel valued and supported. The Service Coordinator will serve as a trusted advocate and resource for residents living in Matthew's Place HUD 202 Senior Housing community.

Requirements

  • Bachelor's degree in Social Work or a related field OR Minimum of five (5) years of experience in social services.
  • Strong communication, relationship-building, and problem-solving skills.
  • Highly organized and able to manage multiple priorities.
  • Demonstrates empathy, professionalism, and sound judgment.
  • Enjoys collaborating with community partners and interdisciplinary teams.
  • Passionate about helping people access the resources they need to live independently.

Nice To Haves

  • At least one (1) year of experience working with older adults or in a geriatric setting.

Responsibilities

  • Conduct assessments to identify residents' social, health, and support needs.
  • Provide case management, resource referrals, and service coordination.
  • Connect residents with community services such as transportation, meal programs, counseling, wellness programs, legal resources, personal assistance, and healthcare services.
  • Build and maintain strong relationships with local agencies, service providers, and community partners.
  • Advocate for residents and help them navigate applications, services, and available benefits.
  • Develop individualized service plans and monitor ongoing support services.
  • Help residents strengthen relationships with family, friends, neighbors, and community resources.
  • Coordinate educational programs and training that support successful aging in place.
  • Recruit and develop volunteer opportunities that enhance resident engagement and well-being.
  • Partner with staff to promote a resident-centered environment that values dignity, independence, and quality of life.
