About The Position

Upper Valley Haven is seeking a Service Coordinator to join their supportive housing team. This role focuses on a caseload of 15 households in the A Way Home Permanent Supportive Housing program in White River Junction, Vermont. The Service Coordinator will help individuals and families maintain stable housing, pursue personal goals, and access necessary services and resources. Unlike many case management positions, this role is not constrained by billing requirements, allowing for creative, flexible, and responsive work to meet tenant needs. Upper Valley Haven is a community-centered nonprofit dedicated to helping people move out of homelessness and hunger toward self-sufficiency through shelter, food access, education, and supportive services, grounded in dignity, equity, respect, and compassion.

Requirements

  • Master's degree in social work, human services, counseling, psychology, or a related field. A bachelor’s degree accompanied by considerable experience can be considered.
  • Experience working with individuals and families with complex needs
  • Knowledge of homelessness, mental health, substance use, and housing stability challenges
  • Strong crisis management and advocacy skills
  • Valid driver's license
  • Relationship-centered, strengths-based, and trauma-informed approach
  • Skilled at building rapport with individuals from diverse backgrounds
  • Experienced working with people facing homelessness, housing instability, mental health challenges, and substance use disorders
  • Creative, flexible, resilient, and solutions-oriented
  • Comfortable balancing independent work with close collaboration
  • Highly organized and committed to timely, accurate documentation
  • An effective advocate who can navigate community systems and partnerships
  • Committed to maintaining the highest ethical and confidentiality standards

Nice To Haves

  • Experience in housing services, case management, or permanent supportive housing
  • Familiarity with HMIS or similar database systems
  • Knowledge of resources and services available throughout the Upper Valley region

Responsibilities

  • Maintain a caseload of approximately 15 households living in permanent supportive housing
  • Build trusting, strengths-based relationships with tenants over time
  • Conduct home visits and community-based meetings
  • Develop and implement individualized service plans
  • Coordinate services with healthcare providers, community agencies, and partner organizations
  • Provide crisis intervention, advocacy, and housing stability support
  • Assist tenants in navigating benefits, community resources, and complex systems
  • Support community-building activities and tenant engagement
  • Maintain accurate and timely documentation and case records
  • Contribute to program development, service improvements, and community partnerships
