About The Position

The Service Coordinator I assumes responsibility for coordinating programs and services to help participants maintain a good quality of life and age in place, prioritizing a person-centric model of service delivery. This role involves acting as a liaison to community agencies, managing supportive services, developing resource directories, organizing educational events, and assisting participants in building informal support networks. The Service Coordinator also engages volunteers and provides individual support through assessments, interventions, and monitoring, ensuring compliance with program requirements such as HUD. The position requires accurate documentation, timely communication, and the ability to manage time independently while working autonomously and as part of a team.

Requirements

  • High School Diploma or GED required.
  • Two or more years of experience in social service delivery with elderly, people with disabilities, and/or family population.
  • Demonstrated working knowledge of supportive services and other resources in the area served by the project.
  • Demonstrated ability to advocate, organize, problem-solve, and provide results for the participants served.
  • Good communication, comprehension and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite and World Wide Web, demonstrated by a passing score on a computer literacy test.
  • Ability to apply common sense understanding to carry out detailed instructions.
  • Ability to perform math skills including adding, subtracting, multiplying, and dividing; to perform the four basic arithmetic operations with money; to perform operations with the assistance of a calculator or Excel spreadsheet.
  • Ability to speak, read, write and understand English.

  • A bachelor’s degree in social work or a related field is preferred.

Responsibilities

  • Coordinate programs and activities for eligible individuals on a group basis.
  • Serve as a liaison to community agencies, network with community service providers, and seek out new services.
  • Monitor the ongoing provision of services from community agencies and document participant usage.
  • Manage the provision of supportive services where appropriate.
  • Develop and maintain a Resource Directory of state and/or local service providers.
  • Organize educational events related to health care, agency support, life skills, and referral sources.
  • Identify partners to provide health services and screenings to participants at their home.
  • Assist participants in building informal support networks.
  • Engage volunteers within the community and/or property when appropriate.
  • Coordinate services for participants on an individual basis through a person-centered, non-clinical assessment, intervention, and monitoring process.
  • Outreach to eligible individuals to provide program education and offer enrollment.
  • Regularly engage participants to identify needs and make referrals to community agencies.
  • Conduct telephonic and/or in-person participant assessments and screenings.
  • Collaborate and communicate appropriate information with care and service colleagues.
  • Provide quality customer service to eligible individuals.
  • Assist participants in acquiring and utilizing desired community services.
  • Adhere to a follow-up and monitoring schedule.
  • Perform miscellaneous and specially requested tasks as needed.
  • Build rapport with participants while maintaining appropriate professional boundaries.
  • Manage time independently, completing required tasks and documentation within identified time frames.
  • Respond to all communications in a timely and professional manner.
  • Accurately record labor time according to program and employer policy and procedures.
  • Complete timely electronic and written documentation within required systems.
  • Complete all required forms with participants who want to utilize the Service Coordination program.
  • Complete Care Plans and monitor plans with participants, including follow-up on referrals and services.
  • Submit documentation needed for funder and quality Performance Reports.
