The Service Coordinator I, according to prescribed policies and procedures of the organization, assumes responsibility for coordinating programs and services to help participants maintain a good quality of life and age in place. This role prioritizes a person-centric model of service delivery. The Service Coordinator will work with eligible individuals on a group and individual basis, acting as a liaison to community agencies, developing resource directories, organizing educational events, and assisting participants in building informal support networks. They will also conduct assessments, collaborate with colleagues, provide customer service, and assist participants in acquiring and utilizing community services to address social determinants of health. The role involves timely documentation, adherence to program requirements, and maintaining professional boundaries.
