About The Position

In Erie County hundreds of individuals and families experience chronic homelessness each year, while many others face ongoing housing instability. Behind every number is a person in need of stability, dignity, and support. UPMC Western Behavioral Health at Safe Harbor is growing and seeking a compassionate, motivated Service Coordinator I to join our Housing Case Management team. In this role, you won’t just support individuals, you’ll be on the front lines of change, helping people rebuild their lives and rediscover hope. At the heart of this work is a nationally recognized, evidence-based Housing First model that follows the approach of providing immediate, permanent, low barrier housing to people experiencing homelessness, with supportive services offered voluntarily. Safe, stable housing is the foundation for lasting wellness and meaningful recovery grounded in the powerful belief that housing is healthcare. Through strong partnerships across the community, our Housing Case Management program is committed to ending homelessness in our county by providing housing navigation and placement support and ongoing, enhanced case management. Working alongside UPMC Health Plan’s Community Team, housing providers, and community agencies and providers, you’ll support individuals facing housing instability, including those living on the streets, in shelters, couch surfing, or at risk of losing their housing. Our program also prioritizes those who are fleeing domestic or interpersonal violence and individuals whose health is directly impacted by their living conditions.

Requirements

  • Must have a completed bachelor's degree (pending graduates welcome to apply!)
  • Must hold a valid driver's license & be agreeable to transporting clients in an agency vehicle
  • Bachelors Degree in a Human Services field such as Social Work, Psychology or Counseling required.
  • Other fields of study may be considered with coursework of at least 12 credits in Human Services.
  • Safe driving record required.
  • ATTENTION: BACHELOR LEVEL TRANSCRIPTS MUST BE ATTACHED WIHT APPLICATION FOR CONSIDERATION.
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Cardiopulmonary Resuscitation (CPR)

  • Act 31 Child Abuse Reporting with renewal
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal

Nice To Haves

  • Preferred experience: case management, mental health, substance use, and/or work with homeless or housing unstable individuals
  • One-year related experience preferred.
  • Prior experience working in community settings preferred.

Responsibilities

  • Coordinates mental health services, leading advocacy and promoting recovery through independent judgment and discretion.
  • Develop and implement comprehensive service plans in collaboration with individuals, families and support systems.
  • Facilitates interdisciplinary meetings, ensuring individuals needs are met and services are accessible.
  • Provide support connecting individuals with internal and external resources.
  • Maintain accurate records, participate in training and stay up to date on best practices and recovery-orientated services.
