About The Position

The Service Coordinator I, a non-exempt position reporting to the Property Manager/Portfolio Manager for Enriched Housing Services/Program Manager, is responsible for coordinating programs and services to help participants maintain a good quality of life and age in place, prioritizing a person-centric model of service delivery. This role involves managing a caseload of up to 1.25 assigned units/individuals per scheduled hours per pay period and adhering to National Church Residences' Core Values.

Requirements

  • High School Diploma or GED required.
  • Two or more years of experience in social service delivery with elderly, people with disabilities, and/or family populations.
  • Demonstrated working knowledge of supportive services and other resources in the area served by the project.
  • Demonstrated ability to advocate, organize, problem-solve, and provide results for participants.
  • Good communication, comprehension, and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite, and World Wide Web, demonstrated by a passing score on a computer literacy test.
  • Ability to apply common sense understanding to carry out detailed instructions.
  • Ability to perform math skills including adding, subtracting, multiplying, and dividing; perform basic arithmetic operations with money; perform operations with the assistance of a calculator or Excel spreadsheet.
  • Ability to speak, read, write, and understand English.
  • Occasional travel as directed by contract and department requests.
  • Must adhere to all policies, procedures, terms, and conditions set forth in the National Church Residences’ Employee Information Guide (EIG) and any facility handbook.
  • Ensure all care is provided with respect and dignity for individuals.
  • Report all complaints made by individuals and/or families to appropriate supervisors.
  • Report all allegations of abuse, misappropriation of funds/individual property, and/or any other corporate compliance, drug-free workplace, safe work practices, and all federal, state, local regulations and laws.
  • Advocate individuals’ personal preferences and right to self-determination.
  • Complete all annual education requirements timely, including Compliance, Privacy, Security, and HIPAA training.
  • Exhibit behavior as set forth by the Code of Conduct in the performance of duties.

Responsibilities

  • Coordinate programs and activities for eligible individuals on a group basis, serving as a liaison to community agencies, networking with service providers, and identifying low-cost options or negotiating discounts.
  • Monitor the ongoing provision of services from community agencies and document participant usage.
  • Develop and maintain a Resource Directory of state and/or local service providers.
  • Organize educational events on topics such as healthcare, agency support, life skills, and referral sources.
  • Identify partners to provide health services and screenings to participants.
  • Assist participants in building informal support networks.
  • Engage volunteers within the community or property.
  • Coordinate services for participants on an individual basis through a person-centered assessment, intervention, and monitoring process.
  • Outreach to eligible individuals to educate them about the program and offer enrollment.
  • Conduct telephonic and/or in-person participant assessments and screenings.
  • Collaborate and communicate with care and service colleagues to achieve participant goals.
  • Provide quality customer service to eligible individuals, answering questions and addressing concerns.
  • Assist participants in acquiring and utilizing community services to address social determinants of health.
  • Adhere to a follow-up and monitoring schedule based on participant preferences and program requirements.
  • Perform miscellaneous and specially requested tasks, such as gathering reports or attending interdisciplinary meetings.
  • Participate in the implementation of grants or special programs.
  • Build rapport with participants while maintaining professional boundaries.
  • Manage time independently, completing tasks and documentation within identified time frames.
  • Respond to all communications in a timely and professional manner.
  • Accurately record labor time according to policy and procedures.
  • Complete timely electronic and written documentation within required systems.
  • Complete all required forms with participants utilizing the Service Coordination program.
  • Complete Care Plans and monitor them with participants, including follow-up on referrals and services.
  • Submit documentation needed for funder and quality Performance Reports.
