About The Position

Assertive Community Treatment (ACT) is a team-based program of medical, behavioral health, and rehabilitative professionals who support individuals with severe and persistent mental illness, primarily schizophrenia, who require intensive services to remain stable. The goal of ACT is to reduce symptoms, prevent relapse, improve daily functioning and quality of life, increase community stability, reduce hospitalizations, and lessen the burden on families. As a Service Coordinator on the Assertive Community Treatment (ACT) Team, you help individuals with severe and persistent mental illness access coordinated, community-based supports that promote stability and recovery.

Requirements

  • Bachelor’s degree in Human Services such as Social Work, Psychology, Counseling
  • One (1) year of case management/service coordination experience with severe and persistent mental illness
  • QMHP registration (or QMHP-trainee registration) within 30 days of hire
  • Successfully pass a criminal history fingerprint background investigation
  • Successfully pass a DMV record check
  • Successfully pass a Child Registry search
  • Successfully pass a drug screening test
  • Successfully pass employment reference checks

Responsibilities

  • Completing intakes and eligibility determinations
  • Developing, updating, and documenting individualized service plans
  • Coordinating referrals and follow-up with providers
  • Advocating for client access to housing, healthcare, benefits, and other resources
  • Supporting discharge planning
  • Participating in team case reviews and related proceedings as needed
  • Connecting with clients, families, and community partners through calls, office visits, and home-based contacts
  • Tracking progress toward current goals and adjusting supports as circumstances change
  • Scheduling appointments, confirming follow-through, and keeping communication clear across care providers
  • Completing timely notes and reports
  • Updating records after contacts
  • Bringing key updates to team discussions
  • Providing crisis support and ensuring next steps are in place before transitioning back to scheduled work
