About The Position

The Service Access Nurse – Cancer Diagnostic Clinic provides services for patients and referring providers seeking care for a suspicion of cancer at Duke. The Service Access Nurse helps establish, promote, and coordinate patient registration, scheduling and record collection for suspicion of cancer patients. The Service Access Nurse acts as a liaison to multidisciplinary and collaborative programs that includes but is not limited to Medical Oncology, Radiation Oncology, Surgical Oncology, Genetics and Radiology. This position will coordinate care between the primary disease group and all appropriate physicians or services. She/he is responsible for ensuring patients receive quality and comprehensive services.

Requirements

  • BSN required
  • Three years of experience in nursing
  • RN licensure and North Carolina State Licensure required.
  • Excellent written, oral and conversational communication skills to effectively work with diverse groups.
  • Ability to analyze, evaluate multiple solutions and solve complex problems using well developed critical & analytical thinking skills.
  • Excellent time management skills needed to prioritize among many competing priorities.
  • Attention to detail needed to operate accurately and effectively.
  • Mastery of Microsoft Office, especially Word and Excel, multiple electronic databases and computer based information systems including eIRB, and EPIC.

Responsibilities

  • Triage undiagnosed cancer referrals to the appropriate disease group
  • Work with Duke physicians and outside physicians to ensure all necessary information is collected, discuss imaging and appointment coordination needs with the oncology providers
  • Ensure all necessary imaging studies are scheduled
  • Handle all incoming e-communications we receive from Duke DPC and ED
  • Review patient records including all relevant medical history and work with appropriate NPCs if additional records are needed
  • Contact referring provider and treating hospitals to assist in obtaining all previous medical records
  • Determine what diagnostic tests and procedures have been completed. In addition, identify necessary tests and procedures needed prior to new patient appointment. Coordinate orders with providers
  • Utilize PowerShare to request outside imaging and utilize eHealth record templates to request outside records, imaging and pathology when appropriate
  • Upload outside records to Epic and other appropriate systems as defined by providers in timely manner and communicate any issues to providers prior to appointments
  • Accountable and responsible for access, facilitation, and coordination of a new patient consults from different sources including phone, fax, e-mails, in-basket messages, and workqueue. Processing referrals may include creating new referrals in EPIC, working from a workqueue, registering new patient accounts, entering insurance information, scheduling and coordinating appointments in some cases
  • Partner with APPs and NPCs to assess patient needs and navigate referrals through the system ensuring proper appointment coordination
  • Oversee complex process often requiring multiple calls to the patient, referring physician, previous facilities/providers to request records and record review with the clinical team
  • Evaluate appropriate level of patient care, utilizing expert level clinical decision making to determine severity of illness for all elective, urgent and emergent referrals
  • Effectively communicate scheduling needs and referral status with patients, families, multidisciplinary providers, support staff, and referring providers within the institution and in the community
  • Have a full understanding of all disease groups, providers, scheduling guidelines and necessary records within disease group
  • Identify additional support services the patient may require during first visit including, but not limited to, social work, nutrition, palliative care and/or survivorship
