About The Position

Our Senior T1D Guide & Trainer provides both non-clinical support to our patients, and mentors and coaches our team of T1D Guides. The Senior T1D Guide & Trainer also partners closely with our care team to train and support staff in providing culturally responsive, empathetic care, ideally in the preferred language of the people we serve. As a trusted guide at a senior level, this role focuses on removing barriers to care and serving as the trusted point of contact for participants as they navigate their diabetes journey. In the Trainer capacity, this role is also a resource to the care team, helping navigate sensitive conversations thoughtfully and engaging with patients in a meaningful, respectful way.

Requirements

  • High level knowledge and understanding of living with Type 1 Diabetes, either personally or with a close family member required.
  • 5+ years of experience working directly with patients living with Type 1 Diabetes.
  • 5+ years of demonstrated experience in training and coaching clinical and non-clinical patient care staff.
  • Strong interpersonal and communication skills, with the ability to work effectively, connect, and understand people in culturally diverse settings.
  • Ability to listen actively, communicate with empathy and gather information in a respectful manner.
  • Ability to connect with patients on their terms and create a psychologically safe environment to facilitate patient empowerment.
  • Ability to use independent judgment to identify when a medical professional should be involved.
  • Ability to communicate information to medical professionals, clinic employees, insurance companies and other community organizations over the phone or virtually.
  • A willingness to accept feedback from clinical supervisors as well as behavioral health staff to ensure provision of high quality coaching.
  • An acceptance of the limits of what coaching entails and a commitment to not overstepping these boundaries.
  • Fluency in English (spoken and written) is required.

Responsibilities

  • Educate, coach and support people with T1D in group and individual settings to achieve better health outcomes. Provide support and guidance from a non-clinical perspective, and partner closely with the clinical care team.
  • Connect participants to BCH and community resources, including insurance navigation, clinical social work, behavioral health and clinical resources.
  • Empower the people we serve to advocate for their health and fully understand the resources available to them.
  • Ensure full and complete documentation of all patient encounters, following standards and protocols.
  • Participate in coordinating care for patients, including appointment tracking, documentation of information, referrals and follow up, facilitating services, and reach-out.
  • Support patients in preparing for upcoming clinical visits, including setting up Zoom with patients and helping patients understand the technology used by our virtual program.
  • Track patient’s engagement and reported progress, and share observations with the Care Team to inform clinical decision making.
  • Assist in setup and syncing of diabetes devices to respective online portals to ensure smooth provision of care with the medical team.
  • Manage the patient communication center in our EHR to ensure patient communications are up to date.
  • Coordinate with the clinical care team by providing non-clinical context and observations to support care planning and service line coordination.
  • Work closely with the clinical care team to identify situations in which medical management may be needed and encourage patients to follow-up with this care.
  • Serve as the primary escalation point for complex, high-need, or sensitive patient situations that require advanced judgment, sustained engagement, or cross-team coordination (while remaining within non-clinical scope).
  • Identify patterns and gaps in patient experience (e.g., engagement drop-off, technology barriers, diabetes distress) and proactively recommend improvements to workflows, resources, or guide practices.
  • Support quality and consistency across the Guide team by reviewing documentation, communication approaches, and patient engagement strategies, providing coaching and feedback as needed.
  • Act as a thought partner and subject-matter resource for the care team on the lived experience of T1D, patient perspective, and non-clinical engagement strategies.
  • Contribute to the development and refinement of program materials, participant resources, and engagement tools to improve patient understanding, trust, and outcomes.
  • Represent the Guide perspective in cross-functional discussions, helping ensure patient needs and lived experience are reflected in care delivery and program design.
  • Support onboarding and ongoing training for care team members on delivering culturally responsive, empathetic and engaging care in a virtual environment.
  • Training on patient engagement and relationship building for care team members.
  • Train and coach care team members on how to empower patients to build confidence, self-advocacy skills, and active engagement in their care.
  • Share best practices for empathetic communication, including active listening, and strengths-based language for people living with T1D.
  • Help the care team recognize and address social, cultural, and systemic factors that impact diabetes management (e.g., health literacy, food access, language barriers, stigma, and technology access).
  • Develop or contribute to communication guidelines, scripts, and scenarios that promote inclusivity, trust, and psychological safety during patient interactions.
  • Coach team members on navigating sensitive conversations (e.g., burnout, diabetes distress, hypoglycemia fear, family dynamics) while staying within clinical scope.
  • Partner with the team to gather patient feedback related to communication and cultural competence, and translate insights into training updates and workflow improvements.
