The Senior Services Case Manager walks alongside seniors and caregivers, providing holistic and thoughtful support and planning. This role involves discovering available services, finding support, and enrolling clients in programs. The case manager will offer individualized, person-centered support, particularly for Holocaust survivors, engaging in comprehensive assessments, care planning, and coaching for clients with complex needs, few natural supports, and barriers to services. The position is highly visible within the community through outreach and engagement, playing a key role in expanding participation in JFCS’s programs for seniors and caregivers. The ideal candidate is compassionate, a good problem solver, an active listener, familiar with trauma-informed care, organized, and self-motivated.
