About The Position

Manages the staff and activities of social work services. Ensures quality assurance and continuous quality improvement of social work services. Ensures staff is informed of changes that impact their work by communicating updates, policy revision, performance outcomes and significant events. Evaluates individual performance and clinical competencies. This leadership role will support the continued growth and expansion of our aging life care and caregiver support programs across the Northwell enterprise.

Requirements

  • Master of Social Work required.
  • Current License to practice as a Clinical Social Worker in New York State required, plus specialized certifications as needed.
  • 6-10 years of relevant experience required.
  • 5-7 years of leadership / management experience required.

Nice To Haves

  • Experience supervising clinical social work or care management teams, highly preferred.
  • Experience in geriatrics, dementia care, caregiver support, palliative care, or aging services, highly preferred.
  • Eligibility for membership under Aging Life Care Professional.
  • Experience with caregiver navigation, hotline operations, or community-based aging programs preferred.

Responsibilities

  • Provides leadership to team members, supervisors and managers of Social Work Services by communicating and guiding toward achieving department objectives.
  • Develops, communicates, and builds consensus for goals in alignment with the health system.
  • Contributes to the development and implementation of strategic vision and plan of the department.
  • Oversees staff development and in-service training to maintain/improve staff performance and ensure the highest quality service delivery possible.
  • Coordinates and provides clinical supervision to social work staff, as applicable.
  • Facilitates training opportunities for staff, and keep appropriate records of required trainings.
  • Reviews and evaluates existing programs, services, practices, and procedures to ensure adherence to policies, Joint Commission Accreditation and New York State Department of Health standards for social work and discharge planning services.
  • Determines staffing needs to ensure adequate availability of services.
  • Develops guidelines for monitoring of social work services to identify opportunities for more efficient and effective delivery of service in accordance with professional, hospital, government, and accrediting agency policies and procedures.
  • Assists with policy development to improve quality of services delivered.
  • Modifies and proposes revision of existing policies and procedures.
  • Recruits, interviews and selects applicants for vacant positions.
  • Coordinates the social work graduate student training program.
  • Participates in internal and external committees and workgroups that require social work expertise and best practice input.
  • Performs related duties, as required.
