About The Position

The Social Work Manager will support the development of the Social Work team within the organization through departmental leadership. This role is responsible for establishing and maintaining social work standards of practice and overseeing quality assurance for social work practices. The manager will also find solutions to issues concerning elders, families, and the facility that impact quality of life and well-being. In addition to maintaining clinical practice quality, this leadership position directly supervises the social work team and collaborates with the Interdisciplinary team to ensure quality care and support the emotional needs of elders.

Requirements

  • Graduate studies in Social Work from an accredited university
  • At least 3 years of experience in the health care industry
  • At least 2 years of experience in an administrative/supervisory position in a social agency
  • Knowledge in discharge planning, SNF, LTC, and Rehab settings
  • Excellent communication and verbal skills
  • Computer literate for MS Word, Excel, and scheduling software
  • Successful leadership of an interdisciplinary team through change
  • Maintains confidentiality

Responsibilities

  • Support and lead the clinical support team (including a social worker, activities, dietician, dining room captains, environmental services, and chaplain).
  • Collaborate with Administration in planning the role of Social Work in leading culture change in our home.
  • Understand the client system, families, elders, staff, and community, and ensure processes are in place to attain commonly agreed upon goals.
  • Contribute to the formulation, review, and amendment of Social Work policies and operational procedures in accordance with the administrative organization of the home.
  • Contribute to Quality Assurance initiatives as an active member in various committees, including the Interdisciplinary Policy and Procedure committee.
  • Act as advisor to the Resident Council and attend monthly meetings. Assist as needed with the election of officers and other identified issues.
  • Prepare the annual budget and initiate procedures to control costs.
  • Maintain statistical and analytical reports of Social Work interventions, complaints, and clinical education as needed.
  • Serve as the Facility grievance officer, responsible for reviewing and maintaining all formal complaints.
  • Provide clinical orientation and training to new social work employees to meet standards in their neighborhood.
  • Facilitate the room change process, working with the neighborhood leadership team to ensure effective room changes that meet the needs of the residents.
