About The Position

This is a Senior Licensed Practical Nurse position working in the Delray Beach Health Center. Work is performed under the supervision of the Community Health Nursing Consultant. This position requires independent judgment, initiative, and the application of general business practices and skills to accomplish various job functions. The position may travel to another Health Center to provide services at the discretion of Administration. This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocol and Procedures. Confidential data sets include Social Security and Clinical information data. The role involves viewing, updating, and releasing information from Health Management System (HMS), Pharmacy Forms System (PFS), and EMDEON. Specific duties include managing client scheduling, service collection, billing, and laboratory test collecting and tracking within HMS. For PFS and EMDEON, the role is critical for dispensing medication by nurse issuance, maintaining a perpetual inventory and audit trail for all medications, filing prescriptions and related documents, ordering and stocking medications, organizing patient-specific medication areas, logging medications, verifying invoices, returning unpicked-up drugs, ordering refills, checking for expired medications, maintaining a quarantine log, packaging and preparing documents for medication transfers, maintaining an adequate supply of drug dispensing forms, stocking Patient Prescription Information (PPI) sheets, issuing clinic medication supplies, and verifying documentation. The position also provides coverage to the medication room at the Annex which services HIV and TB, follows up on patients with recent placement of Long Acting Removable Contraceptives (LARC’s), verifies laboratory and radiology studies, selects and delivers medications for dispensing, administers patient-specific medications, creates and maintains tickler files for delivered medications, makes entries in PFS, prints reports, maintains patient profiles and logs, keeps permits current, assists staff during inspections, and performs clerical duties such as data entry, answering phones, photocopying, filing, and record keeping. The role is responsible for verifying reports on patient charts, obtaining authorizations for visits, assisting with posting maternity patients, ensuring appointments and referrals are made, participating in patient reminder calls, and following up on missed appointments. Additionally, the position reviews laboratory orders, transmits requisitions, conducts reminder calls for laboratory appointments, and follows up on broken appointments. The role is cross-trained to provide services to HIV, Family Planning, and TB programs, and assists with the X-ray protocol. Other duties as assigned.

Requirements

  • Advanced to intermediate computer skills and knowledge of computer programs (Microsoft Word, Excel, E-mail, Internet, Electronic Health Record).

  • Licensure as a Practical Nurse in accordance with Chapter 464, Florida Statutes or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27.
  • Valid Driver’s License.
  • Ability to travel to another Health Center.
  • Must hold and maintain an active Basic Life Support certification.
  • Ability to learn and communicate effectively, orally and in writing, in English.

Responsibilities

  • Provide coverage to the medication room at the Annex for HIV and TB services.
  • Follow up on patients with recent placement of Long Acting Removable Contraceptives (LARC’s).
  • Verify laboratory and radiology studies are completed and results are entered into the medical record.
  • Select and deliver medications for dispensing/issuance.
  • Administer Patient Specific Medications to patients and execute related paperwork.
  • Create and maintain a tickler file for Patient Specific Medication delivered to the Health Center.
  • Prepare items for return to Central Pharmacy, wholesaler, or reverse distributor and complete associated paperwork.
  • Make appropriate entries in PFS and print required reports.
  • Maintain patient profiles, offer to Counsel Signature Logs, and other documents for Central Pharmacy.
  • Keep current permits framed and posted in the medication room.
  • Assist staff during inspections by regulating agencies.
  • Perform clerical duties including data entry, answering the telephone, photocopying, filing, and record keeping.
  • Verify appropriate reports are on the patient chart prior to visit.
  • Obtain authorizations for visits.
  • Assist in posting maternity patients after visits with provider, ensuring appointments and referrals are made.
  • Participate in patient reminder calls and follow-up calls for missed appointments.
  • Review laboratory orders and electronically transmit laboratory requisitions.
  • Conduct reminder calls to clients for their laboratory appointments and follow up on broken appointments.
  • Provide services to HIV, Family Planning, and TB programs.
  • Assist with the X-ray protocol.
  • Perform other duties as assigned.
