About The Position

The list developed from this recruitment will be used for a limited period to fill open and promotional, regular full-time, and part-time positions throughout the County, should they occur. Pursuant to current bargaining unit agreements, employees regularly assigned to the coastal region or in Covelo are eligible to receive a 5% assignment premium. Under direction, performs complex support functions in Health Services. Serves as a vital link between communities and health services to promote health among individuals, families, and groups who lack access to adequate care; determines client financial or medical eligibility for related programs; and may provide childcare service. Relieves professional staff of the more routine duties; offers transportation assistance, processes documents, manages records, and assists with educational materials. Can serve as a bilingual interpreter. Certain assignments will require immediate supervision by a licensed professional. The Senior Integrated Services Coordinator will provide nutrition counseling, customer service, support, and resources to clients, as well as determine family eligibility for the WIC program. The selected candidate will be required to complete the CDPH/WIC Nutrition Assistant certification to provide day-to-day nutrition counseling. Requirements

An associate’s degree related to human services, child development, social or behavioral sciences or a related field AND Two (2) years’ experience performing related functions in community services, client advocacy, outreach work, eligibility assessment, and /or front office duties in a health care setting.
Any combination of education, related experience and training which would provide the required knowledge, skills, and abilities to perform the essential functions of the job.
Valid Driver’s License
Successful completion of WIC Nutrition Assistant (WNA) or Certified Lactation Counselor certifications within one (1) year of appointment.
Incumbents must possess and maintain, or obtain and maintain, all licenses, certifications, registrations, and/or other requirements identified in the minimum qualifications, as applicable, and within the timeframes specified.
Knowledge of applicable state, federal and local ordinances, laws, rules, and regulations.
Knowledge of community based resources including behavioral health, housing, medical, educational support, and other services.
Knowledge of needs, problems, and barriers to health care for low-income and minority populations.
Knowledge of trauma informed care.
Knowledge of process of application for Medi-Cal and other health care coverage.
Knowledge of effects of tobacco, alcohol, and other drugs.
Knowledge of basic screening techniques for tobacco, alcohol, and other drug use and basic techniques for tobacco cessation intervention.
Knowledge of principles and best practices of integrated care, cross-system coordination, and multi-disciplinary service delivery.
Knowledge of office procedures include filing, record keeping, and scheduling appointments.
Knowledge of computer applications and hardware related to the performance of the essential function of the job.
Basic knowledge of early child development.
Knowledge of confidentiality practices and professional boundaries.
Knowledge of cultural sensitivity/responsiveness and effective engagement with diverse populations.
Skill in organizing work, setting priorities, and following up on assignments with a minimum of direction.
Skill in performing case management.

Responsibilities

Provide assistance and support services to clients and licensed/registered health services staff.
Provide case management for medical, nutrition, and social services by coordinating services and making referrals to medical, educational, social, vocational, or preventative programs.
Conduct home visits to assess families and determine appropriate interventions or referrals.
Process and complete assessments and intake forms; determine and re-evaluate eligibility for programs.
Assist in developing immediate solutions to emergency problems and expediting delivery of needed services, may conduct very limited basic health screening and assessments such as weighing, measuring, and taking vital signs.
Act as a liaison between clients, families, and staff by helping schedule appointments, providing interpretation services, and following up on treatment or service plans.
Screen clients for tobacco and substance use; provide education on risks and cessation resources; refer clients for additional substance abuse prevention or treatment services.
Use a strength-based and trauma-informed approach to assess family needs, develop individualized service plans, and provide parent education.
Perform limited supervised testing and basic laboratory work; accurately document results.
Update and maintain client demographic, health, and financial records in accordance with confidentiality and program standards.
Prepare materials and coordinate logistics for group meetings and client education sessions, including creating agendas, preparing documents, and organizing supplies or audiovisual equipment.
Provide on-site childcare during group meetings, including planning and preparing snacks, noting any developmental concerns, and ensuring children's records (e.g., immunizations) are up to date.
Arrange transportation for clients to and from medical appointments or program services.
Maintain accurate client records and support data collection and reporting requirements.
Track and report grant-related expenditures, assist with resource tracking, and maintain program resource materials (e.g., treatment bed availability).
Attend training, in-service meetings, and staff development sessions to maintain and improve knowledge and skills.
Assist in dedicated programs where program specific requirements will be clearly outlined for example Women, Infant, Children’s (WIC) or California Children Services (CCS) or Communicable Disease (CD) or Maternal Child Adolescent Health (MCAH), Behavioral Health and Recovery Services (BHRS) Mobile Services, BHRS Wellness Centers, and other programs that engage in the community with individuals/families to support connection to a variety of services.
Answer incoming calls and greet visitors and the public; provide customer services and support; respond to inquiries and/or directs individuals to the appropriate area or assist them with information, providing applications, or answering questions.
May act as an interpreter and translator when qualified to do so in contacts involving non-English speaking clients.
Perform other related duties as assigned.
