This position coordinates care between numerous healthcare providers, refers patients to providers who can assist them, and shares health and wellness tips relevant to patients. The specialist advocates for patients attempting to make use of public health and government programs, and explains health information in a culturally sensitive, easy-to-understand manner. They advise clients/community groups on issues related to improving general health, such as diet or exercise, and lead support groups, workshops, and seminars on topics of interest to the community. The role involves creating health events for the Florida Department of Health to promote wellness within Nassau County and attending community outreaches to distribute flyers, brochures, and other informational or educational documents to perform outreach and awareness of the Department of Health programs. The specialist maintains accurate records of all patient contacts and protects patient confidentiality. They also develop constructive and cooperative working relationships with others and conduct home visits for targeted population clients to monitor their progress or assess their needs for resources in the paramedicine program.
