About The Position

D’Youville Life & Wellness Community is seeking a Senior Housing Service Coordinator for Bruyere Gardens, its independent living facility. The coordinator works with new and existing tenants to explain the service coordinator program, connect tenants with community services, and support their quality of life. The role also involves maintaining relationships with local agencies, coordinating and monitoring services, facilitating tenant activities and communications, and working with management on tenant conflicts. The listed responsibilities are a brief overview and are not all-inclusive.

Requirements

• Bachelor’s degree or higher in Social Work, Psychology, Gerontology, Counseling, or a related specialty, or significant relevant work experience.
• Basic experience with Microsoft Excel.

Nice To Haves

• Experience with AASC Online Software for the American Association of Service Coordinators.
• Experience working in a HUD housing environment.

Responsibilities

• Explain the service coordinator program to new and existing tenants.
• Identify, assess, select, develop, and maintain referral relationships with local agencies.
• Refer and link tenants to community providers for services including case management, personal assistance, homemaking, meals-on-wheels, transportation, counseling, visiting nurse services, preventive health screening and wellness, and legal advocacy.
• Refer concerns about lease violations to the property Director.
• Develop case plans in coordination with community assessment services.
• Monitor the ongoing provision of community services.
• Facilitate tenant meetings, community organizing, and social activities.
• Support tenants in enhancing their quality of life.
• Work with management to mediate conflicts between tenants.
• Communicate with tenants through meeting minutes, newsletters, flyers, bulletin boards, and similar means.
• Set up a volunteer support program with community service organizations.
