About The Position

The Adult Partial Care (APC) program assists individuals who have severe and persistent mental illness with developing the skills and support necessary to live, learn and work with the least amount of intervention from the behavioral health system. The Adult Partial Care program offers comprehensive, individualized and quality behavioral health treatment within a group setting, designed to promote maximum independence in the least restrictive setting. The Senior counselor has the primary responsibility for leading the team in program in service coordination, provision or arrangement of services needed and coordination of admission/discharge process, personal advocacy and development, review and updating of individual service plans. The counselor provides face-to-face interaction with the consumer supporting the therapeutic contact necessary to achieve the consumer's treatment. The Sr. Counselor interacts with the Program Director formally 1x per week in a supervision/review, and other times as needed for consultation. As an integrated health organization, Acenda values the use of evidence-based practice or EBP's.

Requirements

  • Possess a bachelor's degree in a human services field, or possess a relevant professional credential, such as a CPRP, BSW, certified rehabilitation counselor, CADC, bachelor's degree in rehabilitation, or BSN.
  • At least 2 years of experience with adults with SPMI
  • Must have and maintain a valid driver's license with an acceptable driving record
  • Must be able to exercise independent thinking and good judgment under all circumstances.
  • Must be able to analyze work, set goals, develop plans, and utilize time effectively and efficiently.

Responsibilities

  • Plans and provides group services around consumers' IRP treatment needs. Services include engagement strategies, activities to meet meaningful goals over time, Illness Management and Recovery Program, skill development, prevocational services, counseling, learning activities, social/leisure services; as well as offsite interventions.
  • Maintains authorization timeframes, intake and daily documentation, and service planning.
  • Has primary responsibility for service coordination, provision or arrangement of services needed, personal advocacy, and development, review and updating of individual treatment and service plans.
  • Responsible for timely weekly documentation corrections for billing, coordination of program schedule/changes and ensures records for attendance is documented and complete.
  • Oversees new intakes in getting started, guest visits, incentive programs, socials, and outing schedule/coordination.
  • Facilitates daily meetings with APC team, oversees development of curriculum and meeting required groups.
  • Demonstrates competent written and verbal communication with referral sources and external customers.
  • Attentive and responsive to customer needs in a helpful, supportive and timely manner.
  • Works collaboratively with coworkers and agency staff to effectively communicate and problem-solve.
  • Meets obligations of the program (contractual, licensing, Medicaid, etc.).
  • Follows program policies and procedures and encourages the same in others.
  • Operates conscientiously in accordance with program budget (i.e., use of client engaging funds, office supplies).
  • Performs other related duties as assigned.
